2. I am a person covered by Aspire Student Assistance Program through DeVry University or a family member of such a person.
3. This form is only a request and not authorization for services.
4. Services that can be requested via this form are limited to:
1. Self-Help Information
2. Community Resource Referral
3. Legal Consultation
4. Financial Consultation
5. Counseling Services
5. Response to my request will be sent to the email address I provide within one business day. If I do not hear from Aspire within one business day I should call 1-888-470-1531 to verify that my request was received.
